What EVV captures
The 21st Century Cures Act requires six attributes on every Medicaid-funded personal care or home health visit:
1. Type of service
The HCPCS code or service category being delivered. BridgeCare OS picks this up automatically from the scheduled visit.
2. Date of service
Captured at clock-in. Back-dated entries require supervisor approval and are flagged for audit.
3. Location
GPS latitude/longitude + geofence verification against the patient's home address. Telephony fallback (call-in) for caregivers without smartphones.
4. Individual receiving service
The patient record matched to the visit. Cross-checked against caregiver assignment to prevent visit swaps.
5. Individual providing service
The caregiver record, verified by their authenticated mobile session at clock-in.
6. Time in and out
Captured to the second. Missed clock-outs trigger automatic dispatcher alerts.

How caregivers actually use it
EVV is only as good as the field-staff workflow that captures it. BridgeCare OS ships a mobile-first Progressive Web App that installs on iOS, Android, and Web — so caregivers don't need to download a separate app from the app store.
Clock-in flow
- Caregiver opens BridgeCare OS on their phone, sees today's visits.
- Tap the visit → confirm patient identity → tap clock-in.
- App captures GPS, validates the geofence against the patient address, timestamps the visit.
- EVV record routed to the right state aggregator in the background. No extra step for the caregiver.
Clock-out flow
- Caregiver completes care documentation (or marks it for later if the agency allows).
- Tap clock-out → GPS + timestamp captured.
- If the caregiver forgets, BridgeCare OS auto-alerts them and the dispatcher 15 minutes after the scheduled visit end.
Telephony fallback
Caregivers without smartphones (or in areas with no signal) can clock in via a phone call to a dedicated toll-free EVV line. The caller-ID + recorded PIN authenticates them; the call timestamp captures the EVV data.
Exception handling
Real-world home care has exceptions. EVV mandates don't disappear because the caregiver had a low battery or the patient was at the doctor instead of home. BridgeCare OS handles the common exceptions:
- Missed clock-in: dispatcher can record a manual EVV with the reason; flagged in the audit trail.
- Visit outside geofence: caregiver records the actual location with a brief justification (community outing, doctor's visit, etc.); supervisor approval required.
